CSS style and HTML for contact form 7 Checkboxes

HI,
I would love to know how to get the correct HTML and possibly CSS for the WordPress theme’s Contact Form 7 changes. I really need checkboxes to work but the theme lacks documentation about creating contact forms.
 
please help me ASAP.
